Valoración del metabolismo óseo en pacientes con artritis reumatoide y polimialgia reumática de inicio en el anciano

Abstract

Late-onset rheumatoid arthritis and polymyalgia rheumatica are prevalent pathologies among the population over 60 years of age, a population that is becoming increasingly. We know that both pathologies can be confused from the clinical point of view and both present an increased risk of osteoporosis and bone deterioration, but they are different from the etiopathogenic point of view. In this thesis, the study of bone metabolism in both pathologies is presented, analyzing especially the behavior of bone remodeling mediators. The research of the thesis includes both the clinical part of both pathologies, and the analytical study of bone metabolism and bone quality by densitometry and the measurement of the mediators of bone remodelling, densitometry and the measurement of the trabecular bone score, compared with a healthy control group. Some of the results may have clinical implications, it seems that sCTX may be a predictor of osteoporosis in patients with rheumatoid arthritis of onset in the elderly, and we can establish etiological differences between patients with rheumatoid arthritis and patients with osteoporosis and we can establish etiopathogenic differences between both pathologies by observing the difference in the behavior of bone remodeling mediators. This is the first study on the behavior of bone remodeling mediators in the elderly population with rheumatoid arthritis and polymyalgia rheumatica, helping us to understand the pathophysiology of both diseases and their possible effects at the bone level.
