Trastornos tiroideos e infecciones oportunistas: estudio de dos situaciones complejas en pacientes con miopatía inflamatoria idiopática

    Abstract

    The aim of this doctoral thesis is to analyze two complex scenarios in the practical handling of patients affected by an idiopathic inflammatory myopathy._x000D_ Within the first part, a possible involvement of thyroid dysfunction in the natural history of inflammatory myopathy has been analyzed. The prevalence of thyroid dysfunction among our cohort of patients affected by inflammatory myopathy was studied, as well as the role that this dysfunction can play in the exacerbation and the lack of response to the myositis treatment. _x000D_ In addition, the presence of autoimmune thyroid disorders has been linked significantly with cancer among patients the cohort of patients with inflammatory myopathy. Therefore, possible pathogenic links between inflammatory myositis, thyroid dysfunction and cancer are discussed. _x000D_ Within the second part, the study of opportunistic infections among the same cohort of patients affected by an idiopathic inflammatory myopathy has been done. Thus, the prevalence of opportunistic infections has been analyzed, and etiologies, location, clinical presentation of these infections and main risk factors have been collected. Potential impact on disease prognosis due to a delay on the opportunistic infection diagnosis has also been studied._x000D_ Finally, after analyzing risk factors for each type of opportunistic infection and reviewing prophylaxis recommendations in other entities where immunosuppression is a common feature, a proposal has been done regarding prophylaxis for patients affected by an idiopathic inflammatory myopathy.
