Abstract

Today, health care institutions operate in a dynamic environment because of changes in customer expectations, technological intensity and an increasingly aggressive competition. Health care institutions are looking for tools which allow them to face this situation through the generation of competitive advantages. Recently, some health care institutions have begun to implement lean thinking with the objective of facing the dynamism of the environment._x000D_ The current dissertation is composed of three studies which analyze the effects of lean thinking tools implementation on the operational performance of health care institutions and on the generation of dynamic capabilities._x000D_ The first essay is focused on: (1) identifying whether the implementation degree of continuous improvement tools has a positive impact on operational performance, which is measured through cost efficiency, service quality and lead-time, and, (2) determining whether the health care institutions which have implemented lean thinking tools have a greater operational performance when compared with those which have implemented isolated continuous improvement tools._x000D_ In the second essay, the objective is to identify the lean thinking tool with the greatest impact on the operational performance measured in cost efficiency, service quality and lead-time of health care institutions. To perform this study, 150 executives of private for-profit health care institutions which have implemented lean thinking tools were interviewed._x000D_ In the third essay, through the information gathered with 500 executives, a study is performed to analyze whether the implementation degree of lean thinking tools is positively related with the generation of absorptive, innovative and adaptive capabilities; similarly, the presence of the absorptive capability is analyzed to determine whether it enables the capabilities of innovation and adaptation.
