Shared decision-making (SDM) is one of the pillars of the patient-centered clinical interview. Despite the growing evidence on its benefits, its teaching during healthcare training remains limited and poorly standardized. Therefore, this study aims to assess whether the incorporation of an SDM training program into an existing clinical interview course improves the communication skills of family medicine residents._x000D_

Methodology:_x000D_
Design of a pilot, experimental, longitudinal study with a control group. It includes second-year family medicine residents participating in a mandatory clinical interview course. The intervention group receives the same course with an integrated specific module on SDM. All participants conduct a simulated consultation on hypercholesterolemia, which is video-recorded. The simulated interviews are self-assessed using the SDM-Q-9 questionnaire for patients and the SDM-Q-DOC for professionals. The OPTION5 scale is later applied by two independent observers for video analysis. The scale is translated from English to Spanish and preliminarily validated._x000D_

Results:_x000D_
A total of 72 residents participate: n = 32 in the control group and n = 40 in the intervention group. The baseline comparison shows no significant differences between groups. The intervention group shows a marginally significant improvement in the SDM-Q-9 (p = 0.052) compared to the control group, while both the SDM-Q-DOC and the OPTION5 show significant improvements in both groups, with no differences between them. The duration of the consultation remains stable. Weak correlations are observed between the OPTION5 and the consultation duration, as well as between the different questionnaires. The independent evaluation using the OPTION5 scale reveals systematic differences in scoring between observers._x000D_

Conclusions:_x000D_
Integrating SDM into clinical interview training is feasible and contributes to improving the communication skills of family medicine residents. This approach may promote more patient-centered care. The observed improvement does not discriminate between groups, suggesting the need for further modifications in the specific SDM training design. It is recommended that the OPTION5 scale be used by two simultaneous evaluators to ensure greater reliability.
