CARCINOMA MICROPAPILAR INFILTRANTE DE LA MAMA. PRONÓSTICO A LARGO PLAZO.

Student thesis: Doctoral thesis

Abstract

The classification of breast cancer according to histological type is an example of the heterogeneity of this disease. Infiltrating Micropapillary Carcinoma is a histological variant recently described with controversy whether if it really represents a prognostic factor on its own. This is based on the affinity of this histological type for infiltrating vascular spaces and lymph nodes, which would be related a priori, with a decrease in both overall survival and disease-free survival. Retrospective studies have been published examining the prognosis of IMC, however, this continue to generate controversy. Until today, there is no study that includes a large number of patients from a single center and with a long follow-up period. At the Dexeus University Hospital, IMC began to be diagnosed in 1996, shortly after it was described, in a very homogeneous manner and with strict criteria. This type of tumor constitutes a line of research in our Center. In this study we have included 174 cases with strict follow-up and long evolution. Therefore, it constitutes a highly refined series with a long follow-up period. We found that the group of patients with Infiltrating Micropapillary Carcinoma, despite showing a higher rate of metastasis, did not present significant differences in terms of recurrences, overall survival and progression-free survival when compared with a control group of conventional carcinomas of the breast matched by year of diagnosis. In the multivariate analysis we show that, for the same age, tumor size, histological grade and lymph node involvement, overall survival and progression-free survival is the same for the control group as for the group of patients with Infiltrating Micropapillary Carcinoma.
