Abstract
The paper describes the application of smart cards as communication tools in health information systems providing health care professionals access to a patient's medical history, information on the last treatment, etc. The DIABCARD, used as a specific example, provides the specification for a Chip Card Based Medical Information System (CCMIS) for the treatment of patients with chronic diseases. The DIABCARD CCMIS architecture allows an implementation into different environments and is also equipped with a Portable Personal Medical Record (PPMR) on a chip card allowing access to necessary information at any time for an integrative patient treatment in a shared care environment. Data security, protection and privacy have also been integrated.
